Heart failure (HF) is one of the nation’s most costly diseases, both in terms of quality of life as well as dollar expense. In patients with HF, the heart’s pumping action is compromised. Heart failure affects nearly five million Americans, with 550,000 new cases diagnosed each year.

Program Goals

The HF program is targeted to those with moderate to severe heart failure. The program, which is accredited by the National Committee for Quality Assurance (NCQA), has a primary goal of reducing inpatient hospitalizations.

Services provided

The program is delivered primarily through critical care nurses who assist participants through a combination of intervention, monitoring, and education. The program focuses on:

  • Reinforcing the importance of following the doctor’s care plan

  • Managing medication and drug adherence

  • Educating about diet, exercise, and lifestyle

The bottom line

Participants are:

  • 52 percent below national benchmark for emergency room visits

  • 21 percent above national benchmark for vasodilator compliance
